Q1: Is there a way to allow respondents to select more than one option in response to a single question?
Yes. Every checkbox field that is added is its own standalone check box. In order to make them appear as an option altogether for the form user, follow the steps below:

  1. Create an Instruction field with the question in the Field Label or Alternate Label field.
  2. Create a Checkbox field where the Field Label is one of the choices user can select.
  3. Continue to create Checkbox fields for each one of the options you would like to make available to the user.

Q2: I have a radio button field and one of the options is no longer available. Can I remove it?
Yes, you can remove the option (not the entire field) and any form responses with that option selected still remain.

Q3: Can I add the Answers page as a sub-page to my form?
Sub-pages don't support skipping a level in the hierarchy. The hierarchy is Forms/Responses/Answers, so the only Page/Sub Page combinations possible are Forms/Responses, Responses/Answers or Answers/Answers.

Q4: The Form Field with an Instruction field type on my custom form does not show the field label when viewing it on the Portal. Is this by design?
Yes, this is by design. The instruction's field label will be used in reporting, but it will not show on the Portal. A Form Field record with an Instruction field type displays the Alternate Label or Field Values on the Portal.

Q5: Can I remove a question for an active form?

Widgets: If you're using the Custom Form Widget, you can remove a question from an active form by making the question Hidden = True.

Portal: If you're using a custom form in the Portal, you can make the question inactive by changing it to an Instruction field type, removing the Field Values, and adding "<white space>" as the Alternate Label. ("White space" is just a placeholder, so use any word that strikes your fancy.) Putting it in the brackets will prevent the HTML from showing the content, and the colon should be eliminated. But before you run any report or tool (such as the Form Responses Tool), you'll need to change it back to its original state.

Alternatively you can create a "Graveyard" form and assign the field you would like inactivated to that form. Then, if you need to report on past answers, you would need to reassign the form field back to the correct form.

Q6: I am trying to run a Selected Form Responses report and nothing is populating. The report works without issue for some Forms. What can I do to troubleshoot?
Confirm that your Form's Form Field records don't have a Field Label with special characters or HTML. To use HTML or special characters, use the Form Field's Alternate Label field.

Q7: Can I attach an image to my standalone Custom Form?

Widgets: Yes! Standalone forms display the attached image.

Portal: Standalone forms do not display the attached image (unlike Events).

Q8: I have a form and I want to use it for another Event but want to remove some of the fields, how should I do this?
The best practice is to copy the original form, ensuring to check the box to copy the fields associated with the form. Then, delete any fields that are not needed in the new form. This way you do not lose any of the data from the original form.

Q9: Can I attach files to my Custom Form?

Widgets: Yes! Custom Forms do support attachments.

Portal: No. Custom Forms do not support attachments. However, you can host a file somewhere and link to it using Instructions.

Q10: Is MinistryPlatform HIPAA Compliant for Form Usage?
While it is each church's responsibility to interpret and set their own Health Insurance Portability and Accountability Act of 1996 (HIPAA) compliance policies, the Form Responses Page and specifically the Form Answers Page --where any sensitive information would be stored-- are regular database tables that are stored on your server. Thus, compliance would be related to the security level of your Server, as well as the different Security Methods you assign to the records and pages themselves via Security Roles and Record Level Security in your instance of MinistryPlatform.
